Overview: Psychiatric Mental Health Nurse Practitioner (PMHNP) Hybrid Role

Position Description:

The CoCM Nurse Practitioner is responsible for providing consultative psychiatric care to Bluestone Primary Care patients in coordination with primary care and providing consultation to Bluestone Behavioral Health Integration Program.

The Nurse Practitioner supports the collaborative care team—primary care provider, patient, and behavioral health care manager—through regular consultation on an assigned caseload of patients. As the consultant, the Nurse Practitioner advises the primary care provider on appropriate medications and other treatment strategies, also providing the care team with educational support on diagnosing and managing behavioral health conditions. Patient visits are primarily delivered via telehealth.

CoCM

• Provide regularly scheduled (usually weekly) caseload consultation to behavioral health care managers.
• Work with behavioral health care managers to oversee the status and population health of their assigned patient caseload. The psychiatric consultant uses a registry to track all assigned patients and prioritize patients for case review and treatment adjustment.
• Recommend any treatment adjustments through the case review process or in ad hoc consultations, including medication recommendations and other interventions.
• Advise on treatment for patients who may need more intensive or more specialized mental health care, supporting treatment in the medical setting until patients can be engaged in specialized care as appropriate.
• Provide occasional ad hoc telephone consultation to primary care providers for very complex patients or challenging patient situations.

Consultative Psychiatric Care

• Conduct medically necessary psychiatric consultative telehealth visits referred by Bluestone Primary Care
• Provide consultation and direction to primary care for ongoing management of BH patient needs. 
• Complete all required coding and documentation practices per Medicare and Medicaid guidelines. 

ESSENTIAL JOB FUNCTIONS

• Direct evaluation of patients with significant diagnostic or therapeutic challenges who are identified in discussion with the patient’s behavioral health care manager and/or primary care provider. Such consultation may be provided in person or via telemedicine (video or telephone)
• Develop and implement clinical plans of care in collaboration with primary care
• Medication review and reconciliation, order labs as necessary
• Discuss plan of care with facility nurse and family
• Development and delivery of in-service training for primary care-based providers and staff regarding recognition and treatment of behavioral health conditions in primary care
• Rotate responsibility for emergency phone calls during business hours
• Meet best practice and quality requirements

QUALIFICATIONS/REQUIREMENTS

Education/Experience

• Psychiatric nurse practitioner
• At least 2 years professional clinical experience in Family Practice, Internal Medicine, Geriatric training or interest, preferred 
• Demonstrated ability to collaborate effectively in a team setting
• Ability to be licensed to practice medicine in Florida, Minnesota and Wisconsin
• Ability to obtain and maintain DEA certification in Florida, Minnesota or Wisconsin
• This individual must reside in one of states with a Bluestone presence (FL, MN, or WI).
• Must have and maintain a valid State License

Knowledge/Skills/Abilities

• Proven ability to communicate effectively with strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required

Working Conditions

• Work Environment - This job operates in a professional office environment as well as a remote environment. This role routinely uses standard office equipment such as computers, tablets, phones, and copiers.
• Physical Demands - This is primarily a sedentary role working indoors; however, some walking is required. The position would require the ability to occasionally lift up to 20 pounds and bend or stand as necessary. 
• Travel – Up to 25% local travel is required for this position.
